Bilateral Border Zone Infarctions

A 29 y o female brought unconscious with history of massive post partum hemorrhage.
MRI Dw images show bilateral foci of recent ischemia with restricted diffusion involving fronto parietal cortex and right caudate, areas of involvement correspond to cortical and internal border zone.

Bilateral border zone infarcts are very uncommon infarcts and result from global brain hypo perfusion due to hypovolumia secondary to massive blood loss as in this case or sudden hypotension in cardiac arrest.
